Bone health and physical activity in adolescents with juvenile idiopathic arthritis: a cross-sectional case-control study

BackgroundAdolescents with juvenile idiopathic arthritis (JIA) tend to engage in less physical activity than their typically developing peers. Physical activity is essential for bone development and reduced physical activity may detrimentally effect bone health. Thus, we examined differences in total body bone mineral content (BMC) and areal bone mineral density (aBMD) between adolescents with JIA and adolescent controls without JIA. We also examined associations between moderate-to-vigorous physical activity (MVPA), lean mass, and bone outcomes.MethodsParticipants included 21 adolescents with JIA (14 females, 7 males) and 21 sex- and age-matched controls aged 10–20 years. Assessments included: height; weight; triple-single-leg-hop distance (TSLH); MVPA by accelerometry; and total body BMC, aBMD, and lean mass measured using dual X-ray absorptiometry. Height-adjusted z-scores were calculated for BMC and aBMD and used for all analyses. Multiple linear mixed effects models examined group differences in BMC and aBMD, adjusting for sex, maturity, MVPA, TSLH, and lean mass. Participants clusters, based on sex and age (within 18 months), were considered random effects.ResultsAdolescents with JIA had lower total body aBMD z-scores [β (95% CI); -0.58 (-1.10 to -0.07), p = 0.03] and BMC z-scores [-0.47 (-0.91 to -0.03), p = 0.04] compared with controls. Mean daily MVPA was 22.0 min/day lower in adolescents with JIA than controls; however, MVPA was not associated with aBMD [-0.01 (-0.01 to 0.01), p = 0.32] or BMC [0.00 (-0.01 to 0.00), p = 0.39]. Lean mass was positively associated with aBMD [0.05 (0.01 to 0.09) g/cm2, p = 0.03] and BMC [0.06 (0.03 to 0.10) g, p < 0.001].ConclusionAdolescents with JIA had lower total body aBMD and BMC compared with sex- and age-matched controls without JIA. Group differences in bone outcomes were not associated with the lower MVPA participation of adolescents with JIA. Despite this, physical activity should still be encouraged as it promotes physical well-being.

Relationship between Femur Mineral Content and Local Muscle Strength and Mass.

Among the stimuli able to prevent early decreases in bone mineralization, exercise has a noticeable role per se as the source of mechanical stimulus or through lean tissue enlargement by its increasing of tensional stimulus. However, prevention strategies, including exercise, generally do not establish the moment in life when attention should begin to be paid to bone integrity, according to age group- and sex-related differences. Thus, this study analyzed the relationship between variables from the diagnosis of total and regional body composition, muscle strength, and bone mineral content (BMC) of femurs in young adult males. Thirty-four young Caucasian men (24.9 ± 8.6 years) had their body composition and bone density assessed by dual X-ray absorptiometry. The subjects performed a one-repetition maximum test (1-RM) in a bench press, front pulley, seated-row, push press, arm curl, triceps pulley, leg flexion, leg extension, and 45° leg press for the assessment of muscle strength in upper and lower limbs in single- and multi-joint exercises. Lean tissue mass in the trunk and upper and lower limbs were related to femoral BMC (Pearson coefficient ranging from 0.55 to 0.72, p < 0.01), and 1-RM values for different exercises involving both upper and lower limbs also correlated with femoral BMC (Pearson coefficients ranging from 0.34 to 0.46, p < 0.05). Taken together, these correlations suggest that muscle mass and strength are positively linked with the magnitude of femoral mass in men, even in early adulthood. Hence, the importance of an enhanced muscle mass and strength to the health of femoral bones in young adults was highlighted.

Prevalence and influencing factors of muscle mass loss in adults with diabetes and a high body fat percentage: A cross-sectional study.

In adults with type 2 diabetes (T2DM), sarcopenia and obesity are two common body composition issues. We investigated the associated influencing factors of muscle mass loss in obese adults with T2DM, to provide a theoretical basis for the prevention of sarcopenic obesity in patients with T2DM. We recruited 315 participants in this study. The participants underwent body composition assessment and clinical information was collected. Dual-energy X-ray absorptiometry was used to verify the accuracy of the body composition data. Based on their body fat percentage, 189 patients with T2DM were classified as obese. Patients with T2DM and obesity were grouped into the muscle mass loss group and non-muscle mass loss group based on gender. We collected demographic and clinical information about patients with T2DM who were obese, including their age, gender, body mass index (BMI), appendicular skeletal muscle index (ASMI), and body fat percentage (PBF). Among the participants who were obese and had T2DM, 56.61% (107/189) experienced muscle mass loss, with a detection rate of 43.42% (33/76) among females and 65.49% (74/113) among males. Body mass index, fat index, Android fat, Gynoid fat, limb fat, trunk fat, and total body bone mineral content were all lower in the muscle mass loss group compared to the non-muscle mass loss group, regardless of gender (all P< 0.001). Muscle mass loss in obese adults with T2DM was affected by BMI, body fat index, and limb fat. Muscle mass loss is more prevalent in adults with T2DM and a high PBF. Body mass index, body fat index, and limb fat are the protective factors of muscle mass loss in adult patients with T2DM and obesity.

Low Bone Mineral Content: A New Risk Factor for Outcome of Allogeneic Hematopoietic Stem Cell Transplantation in Patients with Hematological Malignancies

CONCLUSIONS There is growing evidence about the influence of body composition (BC) on mortality in various patient groups, including patients with hematological malignancies. The aim of the study was to assess the impact of BC and bone mineral density (BMD) on the outcomes of allogeneic hematopoietic stem cell transplantation (alloHCT). We carried out an observational study involving patients with various hematological malignancies who underwent alloHCT between 2019 and 2022 in our institution. We assessed their total body BMD and BC employing densitometry with the dual-energy x-ray absorptiometry (DXA) method (Horizon A, Hologic, USA, 2017). Total body BMD includes variables such as BMD expressed in T and Z scores and bone mineral content (BMC). In addition, measured and calculated BC was obtained, which included body fat, and fat-free body mass values. The data on patients and disease characteristics, as well as the transplant details and outcomes were prospectively gathered. The study group consisted of 101 patients (49 males, 52 females), with a median age of 56 years (range, 19-69), presenting with various hematological malignancies classified according to Disease Risk Index (DRI) into low (12%), intermediate (56%), high (16%) and very high risk (16%) categories. The patients received transplants from matched related donors (13%) or matched unrelated donors (88%) after conditioning of either low (40%) or intermediate intensity (61%) according to transplant conditioning intensity (TCI) score. The median follow-up time was 24 months (range, 6-50), with an overall survival (OS) rate of 76% (95%CI 66-84) at 2 years. Significantly worse 2-year OS was found in patients with total body BMC (expressed in grams) below the median value vs ≥ median value calculated for each gender separately (65% vs 87%; p=0.015), high/very high DRI, compared to low/intermediate DRI (57% vs 85%; p=0.009), age over 60 years (64% vs 83%; p=0.045), and female patients, compared to male patients (64% vs 88%; p=0.011). In a multivariate Cox model, low total body BMC [HR 2.84 (95%CI 1.09-7.41); p=0.033], high/very high DRI [HR 3.46 (95%CI 1.42-8.41), p=0.006] and female gender [HR 3.29 (95%CI 1.19-9.07); p=0.021] remained the independent risk factors for OS. For the entire study group non-relapse mortality (NRM) and relapse incidence (RI) at 2 years was 10.4% (95%CI 5.4-19.7) and 24.7% (95%CI 16.4-35.3), respectively. The only identified risk factor for NRM was very high DRI [HR 4.55 (95%CI 1.08-19.18); p=0.039]. The factors that significantly impacted RI both in uni- and multivariate model were low total body BMC and high/very high DRI [HR 3.48 (95%CI 1.25-9.66); p=0.016, and HR 2.84 (95%CI 1.03-5.98); p=0.042, respectively]. Our results strongly suggest that low total body BMC determined by densitometry via the DXA method may serve as a new DRI-independent risk factor for worse alloHCT outcomes in patients with hematological malignancies.

Associations between body composition and cardiovascular disease risk in pre- and postmenopausal women

BackgroundMenopause transition is a critical phase of women’s life since body composition and cardiovascular risk factors begin to change during this period. This study investigated the associations between body composition and cardiovascular disease risk (CVDR) in pre (PrMW) and postmenopausal women (PMW).MethodsA community-based cross-sectional study involving 184 PrMW and 166 PMW, selected randomly from Bope-Poddala area in Galle, Sri Lanka was carried out. Total-body fat mass (TBFM, kg), total body skeletal muscle mass (TBSMM, kg), total body bone mineral density (TBBMD, g/cm2) and total body bone mineral content (TBBMC, g) were measured with total body DXA scanner and they were taken as indices of body composition. CVDR was evaluated using Framingham risk score (FRS%) and individual CVDR factors, such as systolic blood pressure (SBP, mmHg), diastolic blood pressure (DBP, mmHg), fasting blood sugar (FBS, mg/dl), total cholesterol, (TC, mg/dl), tryglycerides (TG, mg/dl), high-density lipoprotein (HDL, mg/dl) and low-density lipoprotein (LDL, mg/dl). Correlations between indices of body composition and CVDR factors were assessed with adjusted partial correlation (adjusted for socio-demographic and gynecologic status, age, daily calorie consumption and physical activity level).ResultsMean(SD) age of PrMW and PMW were 42.4(6.0) and 55.8(3.8) years respectively. TBFM correlated with SBP and DBP (r range; 0.15 to 0.21) and TBSMM correlated with SBP, DBP and HDL (r range; − 0.24 to 0.17) only in PrMW (p < 0.05). TBBMD correlated only with FBS in PMW (r; − 0.21, p = 0.01). TBBMC did not show correlations with CVDR factors (p > 0.05). Body composition indices did not show correlations with total CVDR estimated by FRS and in both groups of women (p > 0.05).ConclusionsBoth SBP and DBP are associated with FM and SMM in different ways among PrMW. This association, however, was not seen among PMW. FBS is associated with BMD only in PMW.

Effect of six weeks of resistance training on bone preservation in older adults: a randomized control trial.

It has been established that chronic resistance exercise contributes to positive changes to bone in older adults. This study evaluated the effect of 6weeks of resistance exercise with either elastic bands or dumbbells vs. a control period on bone morphology of older adults. Fifty-seven adults (mean ± SD; age = 66.5 ± 7.09 yrs; height = 165.2 ± 10.6cm; body mass = 74.5 ± 14.6kg) were randomized into three groups (dumbbell, elastic, or control). Participants underwent a total body dual-energy X-ray absorptiometry (DXA) scan for total body and segmental bone mineral content (BMC) and bone mineral density (BMD) before and following 6-week intervention. Age-matched Z-scores for BMD and BMC were recorded. Data were analyzed using two-way repeated measures ANOVAs and 0.05 significance level. BMCarm improved for the dumbbell group (p = 0.016) after the training, with no change in BMD for any group (p > 0.05). Additionally, significant (time x treatment group) interaction (p = 0.024) of age-matched Z-scores indicated an improvement in only the dumbbell group after 6weeks (p = 0.015), with no change in the elastic group despite them having greater Z-scores than the control group. This study is the first to demonstrate acute normative adaptations as dumbbell-based programs may promote positive maintenance of bone metrics over 6weeks, despite the lack of significant change in absolute BMC or BMD. Adults did not lose relative bone mass with acute exercise using dumbbells as the external load applied and this may lead to positive changes following chronic training. There was no bone-related impact from elastic bands, suggesting a weighted load or force produced relative to gravity is beneficial.

Small animal DXA instrument comparison and validation

Several new peripheral dual-energy X-ray absorptiometry (DXA) devices designed for assessment of bone and body composition in rodents have been developed. We compared the performance (accuracy and precision) of two of these devices, the InAlyzer and the iNSiGHT, to those of an established device, the PIXImus. We measured total body bone mineral content (BMC), bone mineral density (BMD), and body composition (lean and fat mass) on the three DXA devices in 18 male C57Bl/6 J mice (6 each of ages 8, 14, and 24 weeks, weighing 22 to 33 g). DXA body composition measures were compared to whole-body nuclear magnetic resonance (NMR) outcomes. BMC of the femur was also compared to ex vivo micro-computed tomography (microCT). Total body BMD from the InAlyzer and iNSiGHT devices was strongly correlated to that from PIXImus (R2 = 0.83 and 0.82, respectively), but was ~25 % higher than PIXImus. Total body BMC measures by InAlyzer were strongly associated with those from PIXImus (R2 = 0.86), whereas those from iNSiGHT were only weakly correlated (R2 = 0.29). Femur BMC from InAlyzer was strongly correlated with microCT outcomes, whereas iNSiGHT was only weakly correlated. InAlyzer and iNSiGHT fat mass measures were very strongly correlated with PIXImus and NMR outcomes (R2 = 0.91 to 0.97), with slightly weaker associations for lean mass (R2 = 0.81 to 0.76). Short-term precision of InAlyzer and iNSiGHT measurements were excellent, and akin to those from the PIXImus for both body composition and bone measures, ranging between 0.39 and 3.2 %. With faster scan times, closed X-ray source and excellent precision, the new devices are both satisfactory replacements for the now discontinued PIXImus system. However, given the accuracy of the bone and body composition measures, the InAlyzer may be preferable for studies where musculoskeletal changes are the main interest.

A novel non-invasive method for predicting bone mineral density and fracture risk using demographic and anthropometric measures

Fractures are costly to treat and can significantly increase morbidity. Although dual-energy x-ray absorptiometry (DEXA) is used to screen at risk people with low bone mineral density (BMD), not all areas have access to one. We sought to create a readily accessible, inexpensive, high-throughput prediction tool for BMD that may identify people at risk of fracture for further evaluation. Anthropometric and demographic data were collected from 492 volunteers (♂275, ♀217; [44 ​± ​20] years; Body Mass Index (BMI) = [27.6 ​± ​6.0] kg/m2) in addition to total body bone mineral content (BMC, kg) and BMD measurements of the spine, pelvis, arms, legs and total body. Multiple-linear-regression with step-wise removal was used to develop a two-step prediction model for BMC followed by BMC. Model selection was determined by the highest adjusted R2, lowest error of estimate, and lowest level of variance inflation (α ​= ​0.05). Height (HTcm), age (years), sexm=1, f=0, %body fat (%fat), fat free mass (FFMkg), fat mass (FMkg), leg length (LLcm), shoulder width (SHWDTHcm), trunk length (TRNKLcm), and pelvis width (PWDTHcm) were observed to be significant predictors in the following two-step model (p ​< ​0.05). Similar models were also developed to predict leg, arm, spine, and pelvis BMD (R2 ​= ​0.796–0.864, p ​< ​0.05). The equations developed here represent promising tools for identifying individuals with low BMD at risk of fracture who would benefit from further evaluation, especially in the resource or time restricted setting.

Vitamin D supplementation for improving bone density in vitamin D–deficient children and adolescents: systematic review and individual participant data meta-analysis of randomized controlled trials

BackgroundVitamin D supplements are widely used for improving bone health in children and adolescents, but their effects in vitamin D–deficient children are unclear. ObjectivesThis study aimed to examine whether the effect of vitamin D supplementation on bone mineral density (BMD) in children and adolescents differs by baseline vitamin D status and estimate the effect in vitamin D–deficient individuals. MethodsThis is a systematic review and individual participant data (IPD) meta-analysis. We searched the Cochrane Central Register of Controlled Trials, MEDLINE, MBASE, CINAHL, AMED, and ISI Web of Science (until May 27, 2020) for randomized controlled trials (RCTs) of vitamin D supplementation reporting bone density outcomes after ≥6 mo in healthy individuals aged 1–19 y. We used two-stage IPD meta-analysis to determine treatment effects on total body bone mineral content and BMD at the hip, femoral neck, lumbar spine, and proximal and distal forearm after 1 y; examine whether effects varied by baseline serum 25-hydroxyvitamin D [25(OH)D] concentration, and estimate treatment effects for each 25(OH)D subgroup. ResultsEleven RCTs were included. Nine comprising 1439 participants provided IPD (86% females, mean baseline 25(OH)D = 36.3 nmol/L). Vitamin D supplementation had a small overall effect on total hip areal BMD (weighted mean difference = 6.8; 95% confidence interval: 0.7, 12.9 mg/cm2; I2 = 7.2%), but no effects on other outcomes. There was no clear evidence of linear or nonlinear interactions between baseline 25(OH)D and treatment; effects were similar in baseline 25(OH)D subgroups (cutoff of 35 or 50 nmol/L). The evidence was of high certainty. ConclusionsClinically important benefits for bone density from 1-y vitamin D supplementation in healthy children and adolescents, regardless of baseline vitamin D status, are unlikely. Aging and Food Restriction Alter Some Indices of Bone Metabolism in Male Rhesus Monkeys (Macaca mulatta)

Food restriction increases life span, reduces aging rate and affects a wide variety of biological functions. In rats, food restriction delays bone growth and reduces bone density and mineral content. We report the effects of aging and long-term (>6.0 y) food restriction on several indices of bone growth and metabolism in rhesus monkeys (Macaca mulatta). Food allotments for controls approximated free access consumption, whereas food-restricted monkeys received 30% less food on a body weight basis. Cross-sectional and longitudinal age effects on serum alkaline phosphatase paralleled those reported for humans. Food restriction induced a significant delay in the developmental decline (to adult levels) in total alkaline phosphatase and significantly suppressed serum interleukin 6 concentrations, particularly in younger monkeys. Also, food restriction slowed skeletal growth, as reflected by shorter crown-rump length, and significantly reduced total body bone mineral content, but not bone mineral density, measured by dual energy X-ray absorptiometry. Analyses of serum parathyroid hormone, calcium, phosphate and osteocalcin concentrations suggested that the effects on skeletal growth were not related to alterations in calcium and phosphate homeostasis or a primary defect in bone formation. These findings suggest that long-term food restriction delays skeletal development in male rhesus monkeys while allowing the development of a reduced but otherwise normal skeleton.

Long-Term Low Ascorbic Acid Intake Reduces Bone Mass in Guinea Pigs

The effect of long-term (1 y) low to excess ascorbic acid (AA) intake on bone mass was evaluated using guinea pigs that were 12–14 d old at the start of the experiment. Dietary AA was low (0.15 g/kg diet) (n = 7), normal (0.50 g/kg) (n = 8) or excess (10 g/kg) (n = 8). After 12 mo, total body bone mineral density (BMD, mg/cm2) and bone mineral content (BMC, g) were determined by dual energy X-ray absorptiometry. Histomorphometric analysis of the cancellous bone of the proximal tibial metaphysis was completed after in vivo dual fluorochrome labeling. Total body BMD of the low AA group was 4.9% lower (P < 0.05), and total body BMC was 12.4% lower (P < 0.05) than in the normal AA group. Total body BMD and BMC were similar in normal and excess AA groups and in the low and excess AA groups. Histomorphometric analysis indicated significantly greater (P < 0.05) double-labeled bone surface, mineralizing surface, and bone formation rate in the low AA guinea pigs compared with the normal AA animals. Thus, there was greater bone turnover in the low AA group than in the normal AA guinea pigs. No differences in histomorphometric endpoints existed between the normal AA and excess AA groups. Long-term AA deficiency, during the period of rapid growth and slower phases of skeletal maturation, resulted in bone abnormalities in adult guinea pig skeletons. Long-term dietary AA excess caused no such abnormalities.

Whole – body vibration training for children with neurological disabilities

Background. Over the years, the concern over bone health deterioration in children with Neurological Disabilities (ND) has caught the interest of the research community. As the current traditional exercise methods are considered too challenging for children with ND, it is essential to seek effective rehabilitation programs with minimum difficulties and movement restrictions for children with disabilities, ultimately improving their muscle and bone health. Therefore, this study was performed to evaluate the potential application of Whole-body Vibration Training (WBVT) as a beneficial and effective approach to improving Bone Mineral Density (BMD), total body Bone Mineral Content (BMC), and lean mass in children with ND. Methodology. The impact of WBVT on children with ND was investigated using a systematic review and meta-analysis approach following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). A database search was conducted to screen and select past literature sources that were published from January 2002 to July 2022 from EBSCO, PubMed, Scopus, and Web of Science databases and met the inclusion and exclusion criteria. Following the quality assessment (PEDro scale and NIH scale) and sensitivity analysis, the Standardised Mean Difference (SMD) was conducted using the STATA 15.1 software with a 95% Confidence interval (95% CI). (PROSPERO registration number: CRD42022343789). Results. Eight studies (four Randomised Controlled Trial (RCT) and four non-RCT studies) were selected, which involved 184 male and 130 female participants. Based on the PEDro scale, all RCT studies were classified as high methodological quality, while the NIH scale rated all non-RCT papers as "Good". In addition, the meta-analysis results indicated that WBVT substantially enhanced femur BMD [(P ＜ 0.01, z = 3.37), SMD (95% CI) = 0.47 (0.20, 0.74)], lumbar spine BMD [(P = 0.02, z = 2.32), SMD (95% CI) = 0.32 (0.05, 0.58)], total body BMC [(P ＜ 0.01, z = 3.42), SMD (95% CI) = 0.29 (0.12, 0.46)] and lean mass [(P ＜ 0.01, z = 2.80), SMD (95% CI) = 0.25 (0.07, 0.42)] of children with ND. However, the effect of WBVT was insignificant on the total body BMD of children with ND [(P = 0.22, z = 1.24), SMD (95% CI) = 0.14 (-0.08, 0.37)]. Conclusion. The meta-analysis demonstrated the significant effect of WBVT on the femur BMD, lumbar spine BMD, total body BMC, and lean mass in children with ND. Hence, WBVT can be suggested as a complementary treatment prescription for children with ND.

RF10 | PMON216 Aromatase Inhibition Fails to Exaggerate Weight Gain and Induce Bone Loss in Ovariectomized Female Marmosets

Abstract Declining serum estradiol (E2) levels during the menopausal transition are associated with increased central adiposity and heightened risk for metabolic disease. Ovarian estradiol, E2, supports female metabolic function. While ovariectomy (OVX) in rodents enables obesity, OVX in nonhuman primates (NHPs) inconsistently alters weight gain. We therefore hypothesized that in female NHPs, extra-ovarian E2 provides key support for metabolic homeostasis. To test this, we employed aromatase inhibition to eliminate extra-ovarian E2 biosynthesis together with diet-induced obesity (DIO) to enhance weight gain. Thirteen adult female marmoset monkeys were OVX and received: (1) E2-containing capsules and daily oral treatments of vehicle (E2; n=5); empty capsules and daily oral treatments of either vehicle (VEH, 1ml vehicle/kg, n=4), or (3) letrozole (LET, 1 mg/kg in 1ml vehicle/kg, n=4). After 6-7 months, VEH and LET compared to E2 females demonstrated increased % body weight gain (p=0.01) and increased caloric intake VEH (p&amp;lt;0.001) and LET (p&amp;lt;0.001) corrected for fat-free mass. Dual energy x-ray absorptiometry (DXA)-determined body composition at 6 months showed no between female group differences in total fat mass or fat mass in validated body regions of interest. Total body (p=0.014), abdominal region (p=0.002) and upper leg region (p=0.025) DXA-determined fat free mass, however, increased ∼5-10% in all female groups. In addition, lumbar spine and total body DXA-determined bone mineral density (BMD) and bone mineral content (BMC) were comparable across all female groups. Relative circulating E2 levels were E2&amp;gt;&amp;gt;&amp;gt;VEH&amp;gt;LET, while those in hypothalamus ranked E2=VEH&amp;gt;LET, confirming aromatase inhibition of local hypothalamic E2 in LET females. Our findings demonstrate ovarian E2 and extra-ovarian E2 depletion induce comparable increases in DIO weight gain without bone loss in female marmoset monkeys and highlight E2 as a key regulator of female metabolic homeostasis in NHPs. Analysis of serum bone turnover markers in female cynomolgus monkeys of different ages.

ObjectiveThe purpose of this study was to examine bone turnover markers, estradiol, parathyroid hormone, and 25 hydroxyvitamin D, in cynomolgus monkeys at different ages to improve our understanding of the changes in bone turnover markers throughout the life cycle of cynomolgus monkeys and to provide a basis for the establishment of a non-human primate model of osteoporosis.MethodsTotal Body Bone Mineral Density and Total Body Bone Mineral Content were measured using Dual-Energy X-Ray Absorptiometry in cynomolgus monkeys at different ages. Serum bone turnover marker' levels were measured using enzyme immunoassays at each age group, and the relationship between bone turnover markers and age was assessed by Spearman rank correlation analysis to investigate the relationship between bone turnover markers and age in female cynomolgus monkeys.ResultsTotal Body Bone Mineral Density in female cynomolgus monkeys peaked at 10 years of age and then formed a plateau that was maintained until old age. Procollagen I Aminoterminal Propeptide, Bone Alkaline Phosphatase, Osteocalcin, and C-Terminal Telopeptide Of Type I Collagen peaked at 1 to 3 years of age and gradually decreased with age, leveling off by 10 years of age. Estradiol, parathyroid hormone, and 25 hydroxyvitamin D, follicle-stimulating hormone, luteinizing hormone, were not significantly different among age groups.ConclusionThis paper provides data on trends in bone turnover markers throughout the life cycle of female cynomolgus monkeys, which are similar to human changes.

Bone deficits in children and youth with type 1 diabetes: A systematic review and meta-analysis

Deficits in bone mineral and weaker bone structure in children with type 1 diabetes (T1D) may contribute to a lifelong risk of fracture. However, there is no meta-analysis comparing bone properties beyond density between children with T1D and typically developing children (TDC). This meta-analysis aimed to assess differences and related factors in bone mineral content (BMC), density, area, micro-architecture and estimated strength between children with T1D and TDC. We systematically searched MEDLINE, Embase, CINAHL, Web of Science, Scopus, Cochrane Library databases, and included 36 in the meta-analysis (2222 children and youth with T1D, 2316 TDC; mean age ≤18 yrs., range 1-24). We estimated standardized mean differences (SMD) using random-effects models and explored the role of age, body size, sex ratio, disease duration, hemoglobin A1c in relation to BMC and areal density (aBMD) SMD using meta-regressions. Children and youth with T1D had lower total body BMC (SMD: -0.21, 95% CI: -0.37 to -0.05), aBMD (-0.30, -0.50 to -0.11); lumbar spine BMC (-0.17, -0.28 to -0.06), aBMD (-0.20, -0.32 to -0.08), bone mineral apparent density (-0.30, -0.48 to -0.13); femoral neck aBMD (-0.21, -0.33 to -0.09); distal radius and tibia trabecular density (-0.38, -0.64 to -0.12 and -0.35, -0.51 to -0.18, respectively) and bone volume fraction (-0.33, -0.56 to -0.09 and -0.37, -0.60 to -0.14, respectively); distal tibia trabecular thickness (-0.41, -0.67 to -0.16); and tibia shaft cortical content (-0.33, -0.56 to -0.10). Advanced age was associated with larger SMD in total body BMC (-0.13, -0.21 to -0.04) and aBMD (-0.09; -0.17 to -0.01) and longer disease duration with larger SMD in total body aBMD (-0.14; -0.24 to -0.04). Children and youth with T1D have lower BMC, aBMD and deficits in trabecular density and micro-architecture. Deficits in BMC and aBMD appeared to increase with age and disease duration. Bone deficits may contribute to fracture risk and require attention in diabetes research and care. STUDY REGISTRATION: PROSPERO (CRD42020200819).

Osteometabolic profile and bone mass in the transition phase: ethnic differences in Brazilians treated with somatropin during childhood

ObjectiveThe main growth hormone action is to promote linear growth increasing protein synthesis stimulation and osteoblastic activity. Peak bone mass extends from adolescence to 30 years of age. Several factors may influence this acquisition and prevent fracture risk in adulthood, such as genetic potential, GH, ethnicity, and lifestyle factors. This study aims to compare bone mass and osteometabolic profile of white and Afro-descendant Brazilian adolescents in the transition phase, who were treated with human recombinant growth hormone in childhood. MethodsThe authors selected 38 adolescents from the Transition Outpatient Clinic of the University of São Paulo. Lumbar spine and total body bone mineral density (BMD) and bone mineral content (BMC), serum calcium, 25-OH-vitamin D and bone markers were analyzed at the rhGH withdrawal. ResultsThe mean age was 16.8 ± 1.6 years. There were 21 Afro-descendant and 17 whites. Thirty-four percent of the sample presented vitamin D insufficiency, 66% inadequate calcium intake and 44.7% physical inactivity. The Afro-descendants showed a lower lumbar spine and total body Z scores than those of the whites (p = 0.04 and p = 0.03, respectively), as well as their mean body weight (p = 0.03). There were no differences in the remaining osteometabolic parameters. ConclusionAs most adolescents had vitamin D insufficiency, low calcium intake, and physical inactivity, calcium, and cholecalciferol supplementation and lifestyle changes should be encouraged. The Brazilian Afro-descendant may be a vulnerable group for low bone mass, requiring special strategies to increase bone accrual and body weight. More studies are necessary to compare ethnic differences in this population.

Characteristics of total body and appendicular bone mineral content and density in Japanese collegiate Sumo wrestlers

The purpose of this study was to explore the characteristics of total body and appendicular bone mineral content (BMC, kg) and bone mineral density (BMD, g/cm2) in Japanese collegiate Sumo wrestlers. A total of 54 male college Sumo wrestlers were recruited for the study and assigned to two groups according to their body weight (middleweight, 85–115 kg, n = 23 and heavyweight, > 115 kg, n = 31). The BMC, BMD, fat mass, and lean soft tissue mass (LSTM) values were measured using dual-energy X-ray absorptiometry for the total body and appendicular regions. Heavyweight Sumo wrestlers had significantly higher body weight, fat mass, and LSTM compared to middleweight Sumo wrestlers. The mean total body and regional BMC values were significantly higher in heavyweight than in middleweight Sumo wrestlers. The total body and leg BMD was significantly higher in heavyweight than in middleweight Sumo wrestlers, and was significantly correlated with body weight, but not arm BMD. The present study indicates that BMC and BMD might not sharply elevate among even heavyweight athletes, although heavier Sumo wrestlers had a greater BMC and BMD.

Effect of standing frames used in real life on bone remodeling in non-walking children with cerebral palsy.

To compare the bone health of children with severe cerebral palsy who use a static standing frame in real life to that of children who do not. A total of 24 children with severe cerebral palsy GMFCS IV & V were included in the study and were divided into two groups: 13 were using a passive standing frame and 11 were not. We performed a single center retrospective cross-sectional study comparing the two groups using dual X-ray absorptiometry data and tests on biological samples, including bone remodeling factors. Total body (less head) bone mineral content was significantly higher in children who used a standing frame for an average of 30min/day. This was confirmed in the lumbar spine. Although the total body bone mineral density (less head and proximal femur) densitometric data were not significantly higher, a positive trend favored the use of a standing frame in the children. Bone resorptive factors (CTX) were higher in the non-standing-frame group, whereas there was no difference among osteoformation factors. No difference infracture historywas found. We show that non-ambulant children with cerebral palsy who use a static standing frame in real life have better bone health, with lower bone resorption, than children who do not. Further studies are needed to determine how standing practice could impact bone mineralization over time in real life and to explore more bone remodeling factors.

Circumference-Based Predictions of Body Fat Revisited: Preliminary Results From a US Marine Corps Body Composition Survey.

Purpose: Body composition assessment methods are dependent on their underlying principles, and assumptions of each method may be affected by age and sex. This study compared an abdominal circumference-focused method of percent body fat estimation (AC %BF) to a criterion method of dual-energy x-ray absorptiometry (DXA), and a comparative assessment with bioelectrical impedance (BIA), in younger (≤30 years) and older (>age 30 years) physically fit (meeting/exceeding annual US Marine Corps fitness testing requirements) men and women. Methods: Fit healthy US Marines (430 men, 179 women; 18–57 years) were assessed for body composition by DXA (iDXA, GE Lunar), anthropometry, and BIA (Quantum IV, RJL Systems). Results: Compared to DXA %BF, male AC %BF underestimated for both ≤30 and >30 years age groups (bias, -2.6 ± 3.7 and -2.5 ± 3.7%); while female AC %BF overestimated for both ≤30 and >30 years age groups (2.3 ± 4.3 and 1.3 ± 4.8%). On an individual basis, lean men and women were overestimated and higher %BF individuals were underestimated. Predictions from BIA were more accurate and reflected less relationship to adiposity for each age and sex group (males: ≤30, 0.4 ± 3.2, >30 years, -0.5 ± 3.5; women: ≤30, 1.4 ± 3.1, >30 years, 0.0 ± 3.3). Total body water (hydration) and bone mineral content (BMC) as a proportion of fat-free mass (FFM) remained consistent across the age range; however, women had a higher proportion of %BMC/FFM than men. Older men and women (>age 30 years) were larger and carried more fat but had similar FFM compared to younger men and women. Conclusion: The AC %BF provides a field expedient method for the US Marine Corps to classify individuals for obesity prevention, but does not provide research-grade quantitative body composition data.

Vitamin D supplementation and body composition changes in collegiate basketball players: a 12-week randomized control trial.

ABSTRACTBackgroundVitamin D promotes bone and muscle growth in non-athletes, suggesting supplementation may be ergogenic in athletes. Our primary aim was to determine if modest Vitamin D supplementation augments favorable body composition changes (increased bone and lean mass and decreased fat mass) and performance in collegiate basketball players following 12 weeks of standardized training.MethodsMembers of a men’s and women’s NCAA D1 Basketball team were recruited. Volunteers were randomized to receive either a weekly 4000 IU Vitamin D3 supplement (D3) or placebo (P) over 12 weeks of standardized pre-season strength training. Pre- and post-measurements included 1) serum 25-hydroxy vitamin D (25(OH)D); 2) body composition variables (total body lean, fat, and bone mass) using dual-energy X-ray absorptiometry (DXA) scans and 3) vertical jump test to assess peak power output. Dietary intake was assessed using Food Frequency questionnaires. Main outcome measures included changes (∆: post-intervention minus pre-intervention) in 25(OH)D, body composition, and performance.ResultsEighteen of the 23 players completed the trial (8 females/10 males). Eight received the placebo (20 ± 1 years; 3 females) while ten received Vitamin D3 (20 ± 2 years; 5 females). Weekly Vitamin D3 supplementation induced non-significant increases (∆) in 25(OH)D (2.6 ± 7.2 vs. −3.5 ± 5.3 ng/mL; p = 0.06), total body bone mineral content (BMC) (73.1 ± 62.5 vs. 84.1 ± 46.5 g; p = 0.68), and total body lean mass (2803.9 ± 1655.4 vs. 4474.5 ± 11,389.8 g; p = 0.03), plus a non-significant change in body fat (−0.5 ± 0.8 vs. −1.1 ± 1.2%; p = 0.19) (Vitamin D3 vs. placebo supplementation groups, respectively). Pre 25(OH)D correlated with both Δ total fat mass (g) (r = 0.65; p = 0.003) and Δ total body fat% (r = 0.56; p = 0.02). No differences were noted in peak power output ∆ between the D3 vs. P group (−127.4 ± 335.4 vs. 50.9 ± 9 W; NS). Participants in the D3 group ingested significantly fewer total calories (−526.2 ± 583.9 vs. −10.0 ± 400 kcals; p = 0.02) than participants in the P group.ConclusionsModest (~517 IU/day) Vitamin D3 supplementation did not enhance favorable changes in total body composition or performance, over 3 months of training, in collegiate basketball players. Weight training provides a robust training stimulus for bone and lean mass accrual, which likely predominates over isolated supplement use with adequate caloric intakes.
